Tricyclic antidepressants (TcAs) are one of the most commonly used drugs in clinical treatment of depression, with a core structure consisting of a seven membered heterocyclic ring in the middle connected to a benzene ring on both sides. Among them, promethazine is the earliest discovered compound with antidepressant effects, and commonly used drugs include clomipramine, amitriptyline, and doxepin. TCAs are mainly metabolized in the liver and participate in metabolic enzymes such as CYP1A2, CYP2D6, CYP3A4, etc. The main metabolism is tricyclic nuclear oxidation, such as hydroxylation of 2 or 10 carbon atoms (catalyzed by CYP2D6), oxidation of fatty side chains, and demethylation of nitrogen atoms (catalyzed by CYP3A4). Due to the influence of various factors on the activity of CYP, there are significant individual differences in the ratio of active metabolites in plasma to the concentration of the original drug.
